The Mechanism — What Actually Happens at a Cellular Level
Non-Focused 40kHz Inertial Cavitation
The Ultralux V4 / Pro 40kHz handpiece emits non-focused (unfocused) ultrasound — energy disperses through tissue in a broad pattern, not concentrated at a focal point. At the fat cell membrane interface, compression and expansion cycles create microbubble formation. These bubbles undergo inertial cavitation — rapid expansion and violent collapse — generating localised pressure waves that rupture fat cell membranes.
Non-Focused vs Focused Ultrasound — The Critical Difference
Non-focused ultrasound (the 40kHz handpiece): energy disperses broadly through tissue. The superficial skin receives maximum energy intensity due to depth-related reduction. Effective for broad subcutaneous fat deposits. Focused ultrasound (High Intensity Focused Ultrasound): energy concentrated at a precise subcutaneous focal point while sparing overlying skin. These are fundamentally different technologies. The Ultralux V4 / Pro uses non-focused cavitation — not High Intensity Focused Ultrasound.
Post-Cavitation: What Happens to the Released Fat
Triglycerides (fats) released from ruptured fat cells enter the fluid between cells, are transported via the lymphatic system, and pass through circulation to the liver for metabolic processing and elimination. This is why liver and kidney function are clinical prerequisites — compromised liver or kidney function cannot safely clear the additional metabolic load. Adequate hydration (2 litres per day) and light exercise post-treatment support clearance.
What to Expect — Before, During and After
Preparing for Your Session
- Hydration: Drink a minimum of 2 litres of water per day for 72 hours before your session. Cavitation bubbles cannot form efficiently in dehydrated tissue.
- Diet the day before: Follow a low-calorie, low-fat diet for 24 hours before treatment. This reduces the fat load on the liver during the processing window immediately after the session.
- No alcohol: Avoid alcohol for at least 48 hours before your session. The liver prioritises processing alcohol above all other tasks — including clearing the fat released by cavitation.
- Pain relief medications: Avoid anti-inflammatory medications such as Nurofen and Naprogesic for at least 48 hours before treatment, as these increase bruising risk.
- Clothing: Wear loose, comfortable clothing over the treatment area.
What the Treatment Feels Like
Most clients describe cavitation as a warm, relaxing sensation — similar to a firm massage with mild warmth. The handpiece moves continuously in circular motions over the treatment area with ultrasound coupling gel applied throughout.
You may hear a faint buzzing or ringing sound in your ears during the session. This is completely normal, harmless, and stops the moment the handpiece is removed. It occurs because the 40 kilohertz frequency sits at the upper edge of the human hearing range. Some clients notice it; others do not register it at all.
No anaesthetic is required. Each treatment area takes 30 minutes. There is no recovery time — you can return to normal activities straight away.
After Your Session
Your body continues processing released fats for over 72 hours. What you do during this window affects your results:
- 2 litres of water per day for 72 hours — supports lymphatic transport of released fats to the liver
- Low-calorie, low-fat diet for 48 hours — prevents circulating fats from being re-stored in remaining fat cells
- No alcohol for 48–72 hours — keeps the liver free to clear mobilised fats
- Light walking or gentle exercise within 24 hours — a randomised controlled trial confirmed significantly greater fat reduction when cavitation was paired with post-session activity (Taha et al. 2021)

Safety, Contraindications & Aftercare
Absolute Contraindications
Liver disease of any kind — including non-alcoholic fatty liver disease (a common and often undiagnosed condition), hepatitis, cirrhosis · Kidney disease or renal impairment · Significant heart or cardiovascular conditions · Pregnancy · Breastfeeding · Active or recent history of cancer · Pacemaker or any implanted electronic device including cochlear implants and neurostimulators · Metal implants in or near the treatment area · History of deep vein thrombosis or blood clotting disorders · Epilepsy · Severely compromised immune system · Open wounds or active skin infections in the treatment area.
Areas Not Suitable for Treatment
Bony prominences — ultrasound reflects off bone, causing discomfort and reducing efficacy. Do not treat directly over ribs, the spine, the bony pelvis, or areas with less than 2 centimetres of subcutaneous fat. Cavitation does not reach visceral fat (fat stored around the internal organs inside the abdomen) — it only affects subcutaneous fat. The head, neck, and joints are not suitable treatment areas.
Essential Aftercare
For 48 hours after every session: drink a minimum of 2 litres of water per day · Follow a low-calorie, low-fat diet — released fats circulate in the bloodstream and can be re-stored in remaining fat cells if caloric intake is high · Avoid alcohol — the liver must process mobilised fats and alcohol competes for the same metabolic pathway · Light walking or exercise encourages lymphatic drainage and is clinically shown to improve results. Skipping aftercare is the primary reason for poor or absent results.
Body Sculpting — Not a Weight Loss Treatment
Cavitation is a precision sculpting tool, not a substitute for weight loss. The scale may move very little. The changes you will see are in your centimetre measurements and how your clothing fits — particularly in areas where stubborn localised fat has not responded to diet and exercise.
Results are most predictable when you are within approximately 10 to 15 kilograms of your goal weight at a stable weight for at least three months. If you are significantly above your goal weight, a weight loss programme first — then cavitation for refinement — will produce better outcomes than cavitation alone.
Cellulite: Many clients notice smoother skin texture as a secondary benefit, particularly on the thighs and outer buttocks. The acoustic shockwave disrupts the fibrous bands beneath the skin that create the dimpled appearance, while reducing fat cell volume in the same area. For best cellulite results, cavitation works most effectively when combined with bipolar radiofrequency skin tightening (Stage 2).
What the Evidence Actually Shows
What is confirmed: Histological evidence of fat cell membrane disruption and triglyceride release (Bani et al. 2013, PMC4174158). Clinically significant reduction of abdominal fat in the same study. Average 1–3 cm circumference reduction per treated area in prospective clinical studies (Nutrients 2022, 14(17):3498). Waist circumference reductions of up to 7.9 cm confirmed via computed tomography scan in a 2025 Korean hospital study (PMC12092633).
What “permanent” means: Ruptured fat cells do not regenerate — this is confirmed. However, remaining fat cells in treated and surrounding areas can still enlarge with sustained excess caloric intake. Weight maintenance after treatment is essential to preserve results.
